Can a sole individual trustee be the same appointor and primary beneficiary?

While this is not exactly ineffective, it is not a recommended set up. It would be better if the trustee is a corporate trustee and to have a line of succession of appointors.

However, this is best assisted by a lawyer or Legal Team together with the original trust deed.
